Managing your Blood Test Appointments Online

Managing your health online isn't always simple. So we have put together a guide to walk you through booking and cancelling your blood tests online through the UHNM Patient Connect online booking system.

To get started on managing your blood tests online, you will need to access the UHNM - Patient Connect website by clicking here.

You will need:

  • Access to the Internet
  • Access to a mobile phone

This online booking procedure is for individuals who have been requested to have a blood test and have received a blood test form.

If you do not have a blood test form, contact the service that requested a blood test from you. 

 

If you would like to BOOK a blood test appointment, follow the instructions below:

  1. At the top of the webpage, click the 'Book an Appointment' button, highlighted in orange.
  2.  Click the 'Staffordshire Phlebotomy' service.
  3. Select the location you wish to attend for your blood test.
  4. Select your preferred date and time. Use the arrows on the screen to view available appointments in the following weeks. If you cannot find an appointment time that suits you, either try a different location or check back later.
  5. Once you have selected an appointment, fill out the patient details. Please note: You will need access to the mobile you use to register to complete the booking process. Your appointment will be reserved for 5 minutes to allow you time to complete your booking.
  6. You will receive a text on the mobile number you used to register with. Type in the passcode that was sent via text message. If you have not received the passcode, click the small grey text below the text box to 'resend passcode'. This will resend the passcode. If you still have not received the passcode, go back and check that you have entered the correct mobile number.
  7. You will now see the appointment details, check that these are correct and click to confirm.
  8. You will receive a text to confirm that your appointment has been booked and an onscreen prompt with display your booking reference, please keep a note of your booking reference.
  9. Attend your appointment.

 

If you would like to CANCEL a blood test appointment online you must have:

  • Booked the appointment you wish to cancel via the online booking system
  • Access to the mobile you used to register your booking
  • Your booking reference

If you did not book the appointment you wish to cancel online, or do not have access to the items listed above, you will need to contact the Staffordshire Blood Test Line on 01782 674242.

This line gets very busy. If you can't get through, please be patient and try again later.

  1. At the top of the webpage, click the 'Cancel Booking' button.
  2. Fill out the patient details and the booking reference. Your booking reference should look like 'PB-000000'.
  3. You will receive a text message to the mobile that was registered when you booked the appointment. Type in the passcode that was sent via text message. If you have not received the passcode, click the small grey text below the text box to 'resend passcode'. This will resend the passcode.
  4. Your appointment details will now show on screen. Check that this is the appointment you wish to cancel.
  5. Click to confirm your cancellation. You will now receive a text to confirm that your appointment has been cancelled. 
  6. You will no longer need to attend this appointment.
